Procedure for Enrollment of Child's Name in Union Council
1. First of all you should note the normal registration which is registration of the name within 60 days i.e. 2 months after the birth of the child which documents or papers you will need.
Write the correct name of the child from home in Urdu and English, it is better if it is clear, then keep these documents with you before going to the Union Council.
Photocopies of ID card of parents
Child's vaccine card or discharge card from the hospital where the child was born is sufficient. Vaccine or immunization card is better.
A copy of the identity card of the child's grandfather is better if you have it as it is required while filling the form.
Normal registration fee Rs 200 + Form fee Rs 200 Total Rs 400. You get the birth registration certificate in 1 week. After that, you can go to Nadra and make water forms for a fee of 50 rupees. Without Birth Registration Certificate B Form or Child Registration Certificate is not made.

2 passport size photographs of the child
1 1 photocopy of ID card of parents
100 rupees affidavit stamp paper. You can get a stamp from any vendor, only if you have the original identity card of your parents, you can get even 1 stamp. The stamp is not issued without the original identity card.

If the child is born at home, it is mandatory to carry the immunization card
The late registration fee is 200 + 200 but can be as high as 1000 rupees.
The age of the child has reached 7 years and now he remembers or is late for any reason, then the case will go to the court, from there the name will be registered according to the order.
